Summary:
The Nurse Program Coordinator is responsible for assessment, coordination of care particularly for medically complex subsets of patients with high medical comorbidities or served by division's subspecialty programs, evaluation, and documentation of communication and care plans for pediatric patients and their families.
Responsibilities:
1. Assesses patient/families physical, emotional, spiritual, psychosocial and learning needs; communicates pertinent information to the team and other health care providers; assess and recognize changes in patient condition and family dynamics.
2. Provides care coordination and care planning for target subpopulations cared for by pediatric otolaryngology and audiology division, i..e SNHL, tracheostomy dependent, dysphagia, cochlear implant/bone anchored hearing aid patients, and complex patients discharged from the hospital who require short or long term follow up in the division.
3. Provides nursing care to target population of patients and documents evaluation and care delivery in patient plan of care.
4. Determines the education needs of nursing personnel, multi-disciplinary team members and others; implements patient and family education in collaboration with the multi-disciplinary team.
5. Provides leadership to the nursing team and acts as a change agent to improve patient care.
6. Serves as a resource to other nurses and healthcare providers with expertise in assigned practice area.
7. Other duties as required.
Other information:
Qualifications
1. Experience in pediatrics is required
2. Experience in communication, teaching skills, and problem-solving skills is required
3. Experience working with all levels within an organization is preferred
4. Experience in healthcare is required
5. Experience in coordinator role is preferred
6. Experience with complex subspecialties or hospital-based units preferred
7. Proficiency in MS Office [Outlook, Excel, Word] or similar software is preferred. Epic software is preferred.
Education and Experience
1. Education: Bachelor's degree is required. Masters Preferred
2. Certification: RN Licensure is required. PALS and ACLS required within 1 year of hire.
3. Years Relevant Experience: Minimum 3 years of nursing experience required
4. Years of experience supervising: None.
