Summary:

The Home Care Medication Access Coordinator plays a vital role in supporting pharmacy operations and patient services within the Home Care Pharmacy, with a focus on patients receiving care through the Hemophilia Treatment Center (HTC). This role ensures medication access and affordability by managing prior authorizations, maintaining compliance documentation, and supporting operational workflows such as inventory and shipping. The coordinator works collaboratively with pharmacists, clinic staff, and patients to improve access to care, support clinical compliance, and contribute to efficient pharmacy operations.

Responsibilities:

Duties and Responsibilities

1. Coordinates third-party prior authorization processes to ensure timely access to medications for HTC patients.

2. Supports clinical documentation and compliance for HTC patients, including recordkeeping in accordance with Hemophilia Alliance and HTC program requirements.

3. Prepares and compounds medications, including intravenous therapy and chemotherapy, under pharmacist supervision.

4. Assists with the filling, packaging, and shipment of home care prescriptions, ensuring medication stability and accuracy.

5. Maintains pharmacy inventory monitors for outdated or recalled medications, and supports stock rotation and reconciliation.

6. Supports pharmacy accreditation activities through documentation, compliance tracking, and audit readiness.

7. Collaborates with pharmacists, physicians, nurses, and patients to resolve medication-related issues and provide timely service.

8. Utilizes pharmacy systems and electronic medical records (EMR) to document patient interactions and medication access efforts.

9. Assists in operational improvements to enhance workflow, efficiency, and patient outcomes within the Home Care Pharmacy.

10. Adheres to federal, state, and institutional policies, including HIPAA, pharmacy law, 340B program requirements, and safety protocols.

Other information:

Technical Expertise

1. Experience in outpatient or home care pharmacy operations is preferred.

2. Knowledge of specialty pharmacy and rare disease states (e.g., hemophilia) is a plus.

3. Strong communication, organization, and problem-solving skills required.

Education and Experience

1. Education: High school diploma or GED required.

2. Licensure: Certified Pharmacy Technician licensure through the Ohio Board of Pharmacy is required.

3. Certification: National pharmacy technician certification (PTCB or equivalent) is required.

4. Years of relevant experience: Three (3) years is preferred.

5. Years of supervisory experience: None
